Volunteer Background Screening Requirement

To ensure the safety and well-being of the children and families we serve, all volunteers at Jack & Jill Center are required to complete a Level 2 background screening through the Care Provider Background Screening Clearinghouse.

As part of this process, volunteers must fill out the official Background Screening Request Form in its entirety. This form is mandated by the Clearinghouse, the Florida Department of Law Enforcement (FDLE), and the Federal Bureau of Investigation (FBI), and collects personal information required to conduct a fingerprint-based background check.

Once completed, please email your signed form to HR@jackandjillcenter.org.
A member of our Human Resources team will follow up with next steps.

Please Note:

  • The form must be filled out completely in order to be processed.

  • Incomplete submissions will delay your ability to volunteer.

  • This screening is required by Florida law for any individual working or volunteering with children in a licensed facility.
