VPK Enrollment Form - Jack & Jill Center

Dear Parents,

We are gearing up for our upcoming 2023-2024 VPK year! If your child turns four years old on or before September 1, 2023, s/he is eligible to enroll. Jack & Jill Center’s VPK program is offered from 9:00 a.m.-12:00 p.m., Monday-Friday. The first step of the registration process is to obtain a copy of your child’s Certificate of Eligibility (COE). We have attached a flyer with detailed information on how to obtain your child’s COE. We also provide wrap-around care to families that need care longer than our 3-hour VPK program for $210.00 weekly. Parents with children enrolled in this program will have care available from 7:00 a.m.-6:00 p.m. which includes Breakfast, Lunch, and Afternoon Snack.

Please be advised there is an annual registration fee of $75.00 that will be charged to your account. The $75.00 registration fee is per family and covers all children currently enrolled at Jack and Jill. To reserve your child’s spot in our VPK Program, please complete and submit this form ASAP. Please drop off a copy of your COE to the front desk at Jack & Jill Center or email to kcapobianco@jackandjillcenter.org. IMPORTANT: If your child will not be returning for our upcoming school year, please complete the form below with his/her estimated last date of attendance. Classroom space is limited and we cannot guarantee space will be available without the completed pre-registration form AND payment of the annual registration fee.
